To solve the large cave shrine puzzle, you must first progress into the fourth chapter of the main storyline. You will come across a large cave where you must press different combinations of symbols to unlock the steel door. These puzzles can be solved by finding the yellow-colored markings on pillars and the walls inside the cave.
Small Cave Shrine Puzzle
You will also come across a small cave where you will have to solve a similar puzzle (as the large cave shrine puzzle). All you have to do is find the yellow markings on the left side of the shrine and the second floor of the cave as well. Once you press the combinations correctly, you can complete the puzzle quickly.
Cave Shrine Puzzle Rewards
Once you can solve the Cave Shrine Puzzle in Resident Evil 4 Remake, you will get both the Apostate’s and Blasphemer’s Head. Use these items at the Mural Cave to get the Church Insignia as a reward.
